Northland Family Care participates with most insurance plans. As part of your visit we will gladly file your claim with the insurance plans that we accept. In order to file the claim efficiently we will need your current insurance card and photo ID for each visit. For patients who have insurance plans that we do not participate in, we provide an itemized receipt at the time of service for you to file with your insurance carrier. You will receive the best benefits if you stay within your network health plan. It is your responsibility to know if we are in or out of your network with your insurance plan.

Most insurance plans require office visit co-pays and we are required to collect these at the time of service. Please remember to bring a valid form of payment. We accept cash, check, MasterCard, Visa and Discover.
